North Korea demands that Chul perform the surgery, holding Hoon's life as collateral. Though the surgery is successful, the father and son, abandoned by South Korea, end up staying in North Korea. Through Chul's arrangement, Hoon goes to Budapest, Hungary, ostensibly to treat Jae-hee. However, Jin-su, suspicious of this move, discovers Jae-hee's existence. Hoon flees with Jae-hee, deliberately stopping her heart temporarily to make it look like she died. Seizing an opportunity, they escape the hospital and attempt to defect to South Korea, but the two are separated...©SBS

Hun continues searching for Jae-hee. Seeing footage of her in a detention center, he approaches a broker to extract her, only to be told everything depends on money. Through a job from fellow defector Jang-i, he goes to Myeong-woo Hospital. Impressed by the ambitious Jaejoong, Sok-chu tries to get Hun to switch the surgeon to Jaejoong. Ultimately, the lawsuit is avoided, and Jaejoong stays at the hospital, but only Hoon is fired. Seok-chu schemes to manipulate Hoon by using his mother, Mi-sook, but Je-hee, sensing this, takes Mi-sook away from the hospital. Hoon returns to the hospital, setting the stage for a third and final showdown to decide Seok-chu's surgical team.©SBS

However, just before the surgery, he decided the implant procedure would be too difficult due to the patient's underlying condition. Suhyun discovers Sunhee's true identity. Witnessing Jehee comforting the deeply wounded Suhyun leaves him with complex feelings. Jehee then negotiates with Sokchu, demanding he assign both himself and Jejoon to the surgical team to ensure the plan's success, and makes him promise to protect Hoon and his mother's safety. ©SBS

Hoon reunites with his mother. He faces another surgical showdown with Jaejoong, but this time the patient is Chi-gyu's sister, A-yong, who is set to marry next month. Learning surgery would make her unable to have children, A-yong refuses until the very end.©SBS

Just as Jaejoong is about to operate on A-yong, Hoon, moved by Chi-gyu's feelings, interrupts the surgery. When Jaejoong is finally assigned to perform the Prime Minister's surgery, the Chairman appoints him as Acting Chairman. Believing the President has fallen into a coma as planned, Sok-chu reveals his scheme, only for the fully conscious President to hear everything. Knowing the plot, the President hands Hoon and Jae-hee over to Sok-chu. As Sok-chu attempts to leave the hospital, he is shot in the chest by Jin-su, but survives thanks to Hoon's surgery.
